Subject: had've

I heard someone speak this phrase today, "I wish had've gone...", in a context

that where "would've" would've been just as appropriate (forgive me, I

couldn't've resisted if I wanted to.)

Was this just a flaw in speech, or is it an observed extension to the

would've, could've, should've set?
